This Facility Request is only for Saint Luke the Evangelist Ministries/Organizations who serve at our parish. THIS IS NOT A RENTAL FORM FOR PERSONAL/OUTSIDE ORGANIZATION REQUESTS.

Thank you for your interest in using space at St. Luke for your meeting or event. Please be advised that this form does not constitute an agreement or permission for your use of the facility. Approval must be granted by the Facilities Manager and the Pastor. We will respond to your request within 2-3 business days with a reservation confirmation or further questions. If you would like to ask a question about your request, please contact Tim Hammell, SLE Facilities Manager.
Please be aware that we cannot guarantee a particular campus facility (or use a facility space on a permanent basis), because of our limited date availability and constant demand for spaces. The best practice for your requests is to:- Make your requests as soon as possible
- Provide the specific dates and times of your requests (instead of just general requests)
- Be flexible to change dates/facilities
